Immunohistochemistry IHC is a powerful microscopy-based technique for visualizing cellular components, for instance proteins or other macromolecules in tissue samples. IHC is also widely used in research where molecules of interest are analyzed to study their roles in both healthy and diseased cells and tissues on the molecular, cellular or tissue level.

Immunohistochemistry IHC is used in histology to detect the presence of a specific protein marker that can assist with accurate tumor classification and diagnosis. This guide illustrates the basic steps used to create an IHC stain.

Prior to IHC, tissues are removed from the patient or animal and either frozen or chemically preserved fixed and then embedded in paraffin wax. In this way, researchers can look at the localization of the target antigens within cellular components while maintaining the original architecture of the surrounding tissue, as shown in the right panel below. For ICC, most, if not all, of the extracellular matrix and other stromal components are removed, leaving only whole cells, as shown in the left panel below.

Basics of immunohistochemistry.

Immunohistochemistry IHC is an important auxiliary method for pathologists in routine diagnostic work as well as in basic and clinical research including exploration of biomarkers, as IHC allows confirmation of target molecule expressions in the context of microenvironment. Although there has been a considerable progress in automation and standardization of IHC, there are still many things to be considered in proper optimization and appropriate interpretation. In this review, we aim to provide possible pitfalls and useful tips for practicing pathologists and residents in pathology training. First, general procedure of IHC is summarized, followed by pitfalls and tips in each step and a summary of troubleshooting. Second, ways to an accurate interpretation of IHC are discussed, with introduction to general quantification and analysis methods. This review is not intended to provide complete information on IHC, but to be used as a basic reference for practice and publication.

Enzyme-conjugated secondary antibodies are then applied and the specific staining can be visualized after adding the enzyme-specific substrate. Occasionally, when weak or no staining is observed, an antigen "unmasking" by enzyme digestion, may be required. The following procedure describes the application of peroxidase or alkaline phosphatase conjugates in the immunohistochemical labeling of formalin-fixed, paraffin-embedded tissue sections. Note: This step is performed only in cases where weak or no staining occurs, or for antigens requiring "unmasking" according to the primary antibody specifications.
